Chapter 15 Service Agreements - 504 Plans

Students who do not qualify for special education services under IDEA Chapter 14 may still be eligible for support under Chapter 15 Section 504 if the disability is shown to limit their success in the educational environment. Medical, physical, and mental disabilities could warrant the development of a 504 plan. A service agreement is developed between the school and the parent and agreed upon on an annual basis.

If there is a need for the development of a 504 plan, the following documents are completed and submitted to director of pupil services and special education. One document is completed by the parent. One document is completed by a physician.  Upon receiving the forms, a meeting will then be convened to develop the plan. Each year following the initial plan, the parent and school will determine continued need of plan and revisions to the plan (if needed). A new plan is signed at the beginning of each school year.
